Credentialed Contingent Workers
How to submit onboarding packets for CW agencies
Credentialing and Contingent Worker onboarding are 2 separate processes and required prior to start.
- Credentialing can take up to 45 - 60 days.
- Contingent Worker onboarding must be done within 30 days of assignment.
To ensure files are reviewed timely, please note:
- All required steps must be fully complete.
- Documents should be sent as pdfs.

Remote candidates are exempt from providing health-related documentation.
On-site candidates must submit ALL required documentation listed below.
Credentialing & Verification Office Requirements (CVO)
- Please complete the below requirements and forward to CVO@sluhn.org
Criminal clearances
Please note: CVO will accept criminal clearances within the past 60 months for credentialing, however, all contingent workers assigned to work at St. Luke's must have criminal clearances completed 45 days prior to starting their assignment at St. Luke's.
PA State Police Background Check
Candidate should register themselves and complete submission.
PA State Child Line Child Abuse Clearance
Candidate should register themselves and complete submission.
FBI fingerprinting through Department of Human Services*
- Register using the official code online (1KG756)
- Supply receipt of fingerprint scanning and original certificate provided by the Department of Human Services.
- Results with a staple have additional pages (at least 3) and required to be sent for review
Reference verification
- Name, title and email address for the following:
- Peer Reference,
- Current Chair/Chief if provider is currently working or Program Director if provider recently completed training.
Curriculum Vitae (CV)
- A complete/up to date (current) resume with MM/YYYY format for beginning and end dates

FBI fingerprinting through Department of Human Services*
- Register using the official code online (1KG756)
- Supply receipt of fingerprint scanning and original certificate provided by the Department of Human Services.
- Results with a staple have additional pages (at least 3) and required to be sent for review
*Remote workers, located outside Pennsylvania or New Jersey: in lieu of the FBI Fingerprinting, a Nationwide search for any crimes (felonies and/or misdemeanors) in addition to the PA State Police & Child Abuse Clearance is acceptable.
Staff Assigned to the Miners Campus, or any Older Adult Behavioral Health Units or Skilled Nursing Facilities:
- Complete the Disclosure form.
- If worker has lived outside the state of Pennsylvania within the past 2 years, additionally complete the Department of Aging Fingerprint Clearance
- If the Department of Aging Fingerprinting is needed:
- Register using the official code online (1KG8RJ)
- Supply receipt of fingerprint scanning and original certificate provided by the Department of Human Services.
- Results with a staple have additional pages (at least 3) and required to be sent for review
Warren Campus Candidates
New Jersey based work requires these additional clearances:
- NJ state criminal clearance - please do a full state wide clearance for any felony and/or misdemeanor.
- NJ Consolidated Debarment Report Click on the below link, select "Medical" then search for the individual's name, print results and include along with all criminal clearances.
- U.S. Department of Treasury Specially Designated Nationals List Click on the below link, search for the individual's name, print results and include along with all criminal clearances.
Certifications
Roles requiring professional certifications are required to be supply (copies not permitted). Scan as 1 pdf and send to your staffing resources licensing specialist.
- Certifications might include the following:
- CPR/BLS
- All RNs are required to have ACLS except those assigned to OR/BHU/SNF
- Respiratory Therapists are required to have
- NBRC
- NRP (Allentown, Anderson & Upper Bucks assignments)
On-site contingent worker health history requirements.
Scan as 1 pdf and send to your staffing resources licensing specialist.
- Supply all required health history as outlined here.
- Complete the TB Questionnaire.
- Submit 9 panel Urine Drug Screen results.
- Note: Screening should be a forensic study completed by a lab approved or certified by Health and Human Services.
