Important Notice for Grand View Health Patients
Access Your Records Before October 20, 2025
If you are are a registered user of the MyGVH.org patient portal, click here to login if you wish to download important personal health records, including visit summaries and test results before October 20, 2025.
For GVH Hospital records before 07/22/25, contact GVH HIM at 215-453-4850.
For Radiology CDs before 07/22/25, call GVH Radiology at 215-453-4505.
You can access your Grand View Health portal until 10/19/2025. Records will not transfer to SLUHN MyChart.
Please note: You cannot upload previous medical records into your MyChart account. We recommend storing any downloaded records in a secure location for future reference. Your doctor will have access to your records for future reference.
Access your records online.
- MyChart is your secure electronic medical record for St. Luke’s University Health Network located in PA & NJ. You can access/download your records and view your completed radiology images all in your MyChart account.
- If you are unable to locate a record, you may request your Medical Records by sending a secure message through MyChart.
- In the MyChart message center, choose the Medical record request option. After your message is received the requested records will be sent to you via your MyChart portal.
- If you experience technical issues, please contact MyChart Support at: 1-866-785-8537, opt 5.
Your Medical Record… Delivered!
Your Doctor Gets Your Images After Your Test/Study
Your medical records are delivered to physicians who practice at St. Luke’s through a secure website accessed only by medical personnel. Your doctor will view your record and high-quality images in the privacy of the office. And, with the assistance of fellowship-trained radiologists, your doctor can tell you what the results mean for your health.
You never have to worry about picking up your record or radiology images or “films.” If a doctor outside of the Network asks you to bring images to an appointment, Medical Records can send them or electronically transfer them directly to the provider. Please have your providers' complete office mailing address, phone and fax number available when contacting Medical Records.

or obtain a form from any patient care unit at a St. Luke’s University Health Network facility.

You can request records from St. Luke’s Medical Records Department:
- Mail the completed Medical Information Release form
- Email the completed Medical Information Release form to: Releaseofinformation@sluhn.org
- Fax the completed Medical Information Release form to: 833-932-1185
- Call the St. Luke's Medical Records Department at 484-526-4719
Patient records can be emailed to them securely with a signed Medical Release with the email address clearly stated on the release. We cannot fax records to personal home faxes.
Patients 18 and over need to authorize the release of their records unless there is a POA on file.
Spouses and significant others also need to authorize the release of their information.
Non-parental patient representatives such as guardians, foster parents, etc. are required to present documentation to receive records.
The Power of Attorney is void after death. The executor of the estate can obtain this information with proper documentation.
Medical Record staff is not permitted to read patient information over the phone.
St. Luke’s physicians can view your images in your medical record and do not need a CD.
If you need records or a radiology CD sent to a doctor outside of the Network, you can call Medical Records at 484-526-4719. Please have your providers' complete office mailing address, phone and fax number available when contacting Medical Records. The CD will be mailed directly, or images will be sent electronically to your doctor for your appointment.
Medical Records can fax or email your records securely to your employer with a signed medical release that states the employer’s information and exactly what you would like released to the employer.
Behavioral Health records require physician authorization for patient release. Please contact the Medical Records Department at 484-526-4719 for assistance.
No, Medical Records will not be available for patients to retrieve records in-person.

Cost
We will send your records back to you upon receipt of the signed record request form.
For requests other than patient care, you will receive a bill from an independent third-party vendor. St. Luke’s charges only the state regulated charge for copies of your medical record.
Processing Time
Authorization Signature for Your Records Release
- Yourself (not your spouse)
- A parent (for patients younger than 18 years of age)
- A minor who has a child
- A minor (age 14-17) with mental health issue. (For mental health records only)
- A minor who has consented to drug or alcohol treatment
- Legal guardian (guardianship documentation is required)
- Power of attorney if patient is unable to sign (legal documentation is required)
- Estate representative for a deceased patient (estate documentation is required)
