How to submit onboarding packets for CW agencies
To ensure files are reviewed timely, please note the below.
- Provide all completed/required documents at one time
- Only PDFs are accepted
- Documents due the Monday prior to start date
- New Jersey (NJ) assignments will need the following completed:
- NJ state criminal clearance
- PA state criminal clearance
- NJ Consolidated Debarment Report
- U.S. Department of Treasury Specially Designated Nationals List

Remote candidates are exempt from providing health-related documentation.
On-site candidates must submit ALL required documentation listed below.

FBI results
- Only results with a staple will have additional pages included (at least 3)
- All pages are required to be sent for review
Criminal clearances (no more than 30 days prior to starting)
PA State Police Background Check
Candidate should register themselves and complete submission.
PA State Child Line Child Abuse Clearance
Candidate should register themselves and complete submission.
FBI fingerprinting guidelines
- Register using the official code online (1KG756)
- Candidates should keep their payment receipt and original certificate provided by the Department of Human Services. This receipt and certificate should be sent to your staffing agency for your records.
Warren Campus Candidates
New Jersey based work requires a New Jersey State clearance be completed. Sanctioned Clearances to completed:
Education verification
If an educational qualification is required for the role:
- You must provide third-party verification of your education
- Diplomas, degrees, or unofficial copies will not be accepted
All primary source verifications must clearly display the completion date of the request.
Reference verification
To align with our network standards, we have adopted the following reference guidelines. While all references are reviewed individually, please adhere to the criteria below whenever possible:
- Two supervisory references are preferred from the candidate’s most recent employers or assignments
- References must come from two separate employers or assignments
Required Reference Details
Each reference should include:
- The start and end dates of the candidate’s employment or assignment
- The candidate’s eligibility for rehire
- If the candidate is currently employed, please clearly indicate this on the reference form
License & certifications verification
If a professional license or certification is required:
- You must submit a primary source verification obtained directly from the issuing authority’s online system.
- Copies of licenses or certifications are not accepted, except under the following condition:
- For Penn Foundation Assignments Only
Both of the following must be submitted: - A copy of the license and/or certification
- The primary source verification, including the date the verification was completed
All primary source verifications must clearly display the completion date of the request.
Only on-site contingent workers need to provide health information. If the worker is fully remote, no health documents need to be sent. This is a general outline; for specific details, please refer to your company’s service agreement.
- Supply all required health history as outlined here.
- Complete the TB Questionnaire.
- Submit Urine Drug Screen results.
Note: Screening should be done by a lab approved or certified by Health and Human Services. This will be part of a forensic study.
