Onboarding Guidance For Contingent Worker (CW) Agencies

How to submit onboarding packets for CW agencies

To ensure files are reviewed timely, please note:

  • Complete all required steps.
  • Provide all documents in 4 pdfs as outlined below.
  • Documents due the Monday prior to start date.
  • Forward all documents to your St. Luke’s Regulatory & Licensing Specialist contact.

Please scan the documents into 4 separate PDFs as outlined below:

Remote candidates are exempt from providing health-related documentation.
On-site candidates must submit ALL required documentation listed below.

Sanctioned Clearances

Click on each of the below links, search for the individual's name, print results and include along with all criminal clearances.

Criminal clearances (no more than 30 days prior to starting)

PA State Police Background Check

Candidate should register themselves and complete submission.

PA State Child Line Child Abuse Clearance
Candidate should register themselves and complete submission.

FBI fingerprinting through Department of Human Services*

  • Register using the official code online (1KG756)
  • Supply receipt of fingerprint scanning and original certificate provided by the Department of Human Services.
  • Results with a staple have additional pages (at least 3) and required to be sent for review

*Remote workers, located outside Pennsylvania or New Jersey: in lieu of the FBI Fingerprinting, a Nationwide search for any crimes (felonies and/or misdemeanors) in addition to the PA State Police & Child Abuse Clearance is acceptable, . 

Staff Assigned to the Miners Campus, or any Older Adult Behavioral Health Units or Skilled Nursing Facilities:

  • Complete the Disclosure form.
    • If worker has lived outside the state of Pennsylvania within the past 2 years, additionally complete the Department of Aging Fingerprint Clearance

  • If the Department of Aging Fingerprinting is needed:
    • Register using the official code online (1KG8RJ)
    • Supply receipt of fingerprint scanning and original certificate provided by the Department of Human Services.
    • Results with a staple have additional pages (at least 3) and required to be sent for review

Warren Campus Candidates

New Jersey based work requires these additional clearances:

 

Education verification

Roles requiring education are to supply either:

  • Third-party verification of education OR
  • Original diploma 

Reference verification

  • Two supervisory references from the candidate’s most recent employers or assignments to include:
    • start and end dates of employment/assignment,
    • eligibility for rehire,
    • if the candidate is currently employed/assigned.

Primary Source Verification (license & certifications)

Roles requiring professional licenses or certifications are required to supply (copies not permitted):

  • Primary source verification for licensure must be obtained directly from the issuing authority’s online system within 30 days of starting at St. Luke's.
  • Certifications might include the following: 
    • CPR/BLS
    • All RNs are required to have ACLS except those assigned to OR/BHU/SNF
    • Respiratory Therapists are required to have
      • NBRC
      • NRP (Allentown, Anderson & Upper Bucks assignments)
  • Penn Foundation Assignments will accept copies of license and/or certification. Primary Source Verification is required.

On-site contingent worker health history requirements. 

  • Supply all required health history as outlined here.
  • Complete the TB Questionnaire.
  • Submit Urine Drug Screen results.
    • Note: Screening should be a forensic study completed by a lab approved or certified by Health and Human Services.

If the worker is fully remote and no patient contact, no health documents or drug screening is required.
