A Special Announcement for those Aged 64 and Older

This is a special announcement from St. Luke’s Warren Campus for those aged 64 or older who are:


  • selecting between traditional Medicare and a Medicare Advantage plan
  • considering changing from traditional Medicare to a Medicare Advantage plan
  • considering switching from one Medicare Advantage plan to another

In addition to the cost considerations of available Medicare coverage options, there are important questions that you should ask if you are considering a Medicare Advantage plan rather than traditional Medicare. (Please be aware that Medicare Advantage plans are NOT to be confused with a Medicare traditional “supplemental” plan).


  1. Will I be able to keep my hospital and physicians if I choose a Medicare Advantage plan?
  2. What will my out-of-pocket expenses be if I choose a Medicare Advantage plan?
  3. Will I need referrals to visit specialists?
  4. Will the Medicare Advantage plan cover me if I get sick while traveling in another state?
  5. What will my prescription medications cost?
  6. Are my medications on the Medicare Advantage plan's covered-drug list (formulary)?
  7. Does the Medicare Advantage plan include the pharmacies I currently use?
  8. Can I get my prescriptions through the mail?
  9. Does the Medicare Advantage plan have a good quality rating? (1-5 Stars with 5 as best)
  10. Does the Medicare Advantage plan cover any services that traditional Medicare does not cover? (ex. vision, dental, fitness membership)

YOU SHOULD ASK THESE QUESTIONS BEFORE YOU CHOOSE A MEDICARE OR MEDICARE ADVANTAGE PLAN. UNDERSTAND THE FACTS BEFORE YOU MAKE A DECISION.

  3. Aetna (except for the Aetna Medicare NNJ Prime Plan HMO)
  4. Horizon (except for the Horizon Medicare Blue Advantage HMO).

IMPORTANT: St. Luke’s Warren Campus and all its affiliated outpatient sites and physician offices are considered “in-network” as part of the Aetna Medicare Advantage PPO ESA plan.
