St. Luke's Family Practice - Forks
I am a family medicine physician at St Luke's University Health Network. My practice is dedicated to providing the best quality care to our patients.
I am a board-certified family physician who treats patients with all medical conditions. As part of a multidisciplinary group, my colleagues and I offer patients a team approach to treating your physical and emotional conditions, as well as preventative care.
I am a family physician with a strong background in Gynecology. Additionally, I provide services for small surgical procedures such as: skin biospies, incision and drainage of abcesses, skin tag removals, simple suturing, etc.
I am a board-certified family physician who joined St. Luke’s University Health Network in 2011. Prior to that I was a part of Warren hospital since 2007 which is also now part of the St Luke’s University Health Network. I focus on providing the best health care possible and creating a strong patient-provider relationship. I work with my patients to provide the best health outcomes and provide my patients with the necessary knowledge to be healthy.
In my spare time I enjoy spending time with my family. I also enjoy reading, cooking and traveling.

“Always great to see Dr. Mariam”
3/25/2025
“Dr. Mariam was my PCP since Dr Nagpal left. I do miss Dr. Nagpal but Dr. Mariam was very professional and thorough. She took the time to go over my medical records and explained and answered any questions I had. I was very pleased with the care Dr. Mariam gave me. It was hard because she had big shoes to fill after losing Dr. Nagpal. I was very pleased with the care I received from Dr. Mariam.”
