St. Luke's Family Practice - Forks
I chose a career in family medicine for the special relationships that are made in primary care. As a family medicine provider, I build trust, learn the goals of each patient and grow with them whether they are at birth or toward the end of life. I enjoy caring for patients of all ages. I had a great role model who showed me the importance of these long lasting, meaningful relationships from a young age. Luckily, he is also part of the St. Luke’s family in New Jersey.
I earned my medical degree from Lake Erie College of Osteopathic Medicine. I completed my residency at Tower Health Reading Hospital and worked in Reading for a year after graduating. I moved back into the Easton area after being pulled back toward my hometown right over the border in New Jersey. I am excited to start a new chapter at St. Luke’s Forks Family Medicine. I am currently board certified by the American Osteopathic Board of Family Physicians.
Outside the office, I love spending time with my wife, Emily and with family. I also have several hobbies including woodworking, wood burning, wood carving and cycling.

“My experience with Dr. Prentiss was a good one. He addressed my severe cough and cold right away, seeing how ill I was, and he explained very well what was going on with my lungs. Dr. Prentiss took my vitals and knew right away what medications I needed. He informed me of what each medication would do for my problem and then he ordered them at my pharmacy. My health has been improving ever since.”

“I have always had the best relationship with Dr. Prentice, the nursing assistants, and clerical staff at St. Luke's Family Practice in Forks Twp., PA. He and his office staff are very responsive to any questions I might have, whether by phone or through MyChart. When our original GP retired two years ago after being our primary care physician for over 30 years, he recommended Dr. Prentice. Now my whole family (husband and two sons) see Dr. Prentice regularly.”

The Patient Satisfaction Rating is an average of all responses to the care provider related questions from our
nationally recognized Press Ganey Patient Satisfaction Survey. Patients that are treated in outpatient or hospital environments may receive different surveys, and the volume of responses will vary by question.
Responses are measured on a scale of 1 to 5 with 5 being the best score.
The comments are submitted by patients and reflect their views and opinions. The comments are not endorsed by and do
