St. Luke’s Physician and AP Awards honor the Network’s outstanding Physicians and Advanced Practitioners (APs) who exemplify the qualities and characteristics of our mission and vision and meet the criteria of each award category.

Nomination Process
- Multiple nominations are allowed for a single Physician or AP, but each submission must be separate.
- Individuals may nominate more than one Physician or AP through separate nominations.
- Any SLUHN employee may nominate a Physician or AP for these awards.
- Group or team nominations are not accepted, but each team member could be nominated separately, as above.
The Excellence in Patient-Centered Care Award recognizes an exceptional Physician and AP who consistently prioritizes patients’ well-being, demonstrates compassion, and goes above and beyond to provide patient-centered care.
One award will be presented to a Physician and one award will be presented to an AP. APs include Nurse Practitioners (NPs), Certified Nurse Midwives (CNMs), Certified Nurse Anesthetists (CRNAs), or Physician Assistants (PA-Cs).
Eligibility Criteria:
The nominees must be employed with SLUHN for a minimum of 1 year.
Criteria for Consideration:
Nominees for the award should exemplify qualities that make them especially deserving of this recommendation. These features include, but are not limited to, the following values and characteristics:
- Empathy and Compassion: They actively listen to patients, understand their unique needs, and treat them with kindness and respect.
- Holistic Approach: They consider not only the medical condition but also the emotional, social, and cultural aspects of each patient. Their care plans address the whole person, promoting overall well-being.
- Patient Education: They empower patients by providing clear explanations, answering questions, and involving them in decision-making.
- Education promotes informed choices and active participation in health management.
The Excellence in Teamwork and Collaboration Award recognizes an exceptional Physician and AP who consistently demonstrates outstanding teamwork, fosters collaboration, and contributes to a positive work environment.
One award will be presented to a Physician and one award will be presented to an AP. APs include Nurse Practitioners (NPs), Certified Nurse Midwives (CNMs), Certified Nurse Anesthetists (CRNAs), or Physician Assistants (PA-Cs).

- Supportive Attitude: They uplift colleagues, celebrate successes, and provide constructive feedback. Their encouragement fosters a sense of camaraderie and mutual respect.
- Collective Problem-Solving: They approach challenges as a team, leveraging collective expertise. Their collaborative problem-solving leads to innovative solutions.
Department Nominated Awards
We’re excited to announce that this year’s Physician of the Year and Advanced Practitioner of the Year awards will be nominated by each department.
Nomination Process
- Each department will submit a single nomination for a Physician and a separate single nomination for an AP.
- The physician and/or administrative leader will coordinate with their teams to vote and submit their nominations.
- Nominations forms will be sent directly to department leaders.
This award recognizes an exceptional Physician whose work and clinical practice exemplify unwavering commitment to SLUHN’s vision, mission, and goals. The recipient serves as a role model, mentor, and collaborative partner across the entire spectrum of healthcare delivery, distinguishing themselves as a leading influencer in their profession.
Eligibility Criteria:
- The Physician must have been employed with SLUHN for a minimum of 1 year.
- They should have at least 5 years of practice experience.
Criteria for Consideration:
Nominees for this award should embody qualities that make them especially deserving of recognition, including:
- Inspiring Others: Demonstrating the ability to motivate and uplift colleagues.
- Caring and Betterment: Dedication to improving the well-being of all individuals.
- Community Engagement: Active participation in medical and community service.
- Clinical Excellence: Exceptional knowledge and skill in patient care.
- Patient and Team Relationships: Building strong connections with patients and fellow team members.
- Alignment with SLUHN’s PCRAFT Values: Living out the organization’s values.
- Teaching and Mentorship: Guiding and shaping the next generation of healthcare professionals.
- Team Building: Fostering collaboration and unity.
- Leadership and Education: Advancing knowledge and expertise through leadership and education.
- Research Contributions: Demonstrating a commitment to advancing medical knowledge.
This award recognizes an exceptional Advanced Practitioner (APs) whose clinical practice and dedication align with the vision, mission, and goals of SLUHN. The recipient serves as a role model, mentor, and collaborative partner across the entire spectrum of healthcare delivery. Their influence extends beyond their peers, making a significant impact in their profession.
Eligibility Criteria:
- The AP must have been employed with SLUHN for a minimum of 1 year.
- They should have at least 5 years of practice experience in an AP role.
Criteria for Consideration:
The Advanced Practitioner Awards eligibility includes Nurse Practitioners (NPs), Certified Nurse Midwives (CNMs), Certified Nurse Anesthetists (CRNAs), or Physician Assistants (PA-Cs), who exemplify excellence in the following areas:
- Inspiring Others: Demonstrating the ability to motivate and uplift colleagues.

Selection Process for All Awards
- Nominees will be reviewed by the Provider Development Committee. Committee will use selection criteria to judge the strength of each nominee.
- Names and specialties will be blinded prior to review to prevent bias.
- Top 3 Physician and AP candidates will be reviewed by HR to ensure they are in good standing prior to selection.
- Top 3 Physician candidates will be sent to a Physician review board for final selection.
- Top 3 AP candidates will be sent to AP review board for final selection.
- Recipients cannot win back-to-back years.
