How to Apply
We are accepting applications for the 2026 academic year beginning in August 2025.
To apply for the St. Luke’s EMS Fellowship, please send the following documents to the EMS Fellowship Program Coordinator, Anne Smith at Anne.Smith@sluhn.org with subject “St. Luke’s EMS Fellowship Application”.
The applicant will need:
- 1. An updated CV
- 2. Three Letters of Reference
- a. One must be from your Program Director
- b. One should be from an EMS Physician or EMS Medical Director
- i. An EMS Agency may submit a letter of recommendation as long as the EMS Medical Director is a member of the signature group.
- 3. Letter of Interest/Personal Statement
- 4. USMLE or COMLEX Transcript (an unofficial one will be accepted at this time)
- 5. The filled out EMS application document
