Application materials consists of:

1. Personal information (name, address, contact information)
2. Disclosure of criminal behavior, academic violations, and/or licensure actions
3. Employment history
4. Military service history (if applicable)
5. Education (undergraduate and graduate)
6. Three references (including one from current advisor or department head)
7. Personal statement (max 3000 characters) indicating why you want to go into medical physics
8. Uploaded CV (should include awards, publications and presentations)
9. Transcripts
   1. Can be uploaded, or
   2. Official copies can be mailed to Medical Physics Residency Application, St. Luke’s Anderson Campus, 1872 St. Luke’s Blvd., Easton, PA 18045
10. Candidates must also be graduates of CAMPEP-accredited MS/PhD programs
