St. Luke’s Employer Vaccine Toolkit

St. Luke’s Occupational Medicine team wants to give valued employer partners the most convenient and up to date information, as well as materials for vaccinating employees in a safe and organized way. With guidance from the CDC and DOH, St. Luke’s continues to roll out the “phased” approach recommended that will equitably organize vaccination inventory, patient needs and the needs of the community.

Please share these materials with your employee population. We ask you to encourage your staff to pre-register on the St. Luke’s MyChart application for their FREE COVID-19 vaccination. As state phases change, MyChart will send regular updates, including further vaccine reservation details such as when the employee can schedule and where.

  1. Employee Correspondence Template – Email template for employee population correspondence which will explain the entire vaccination procedure while directing staff to our website for more information.
  2. MyChart Instructions – Instructions with visual explanation on how to set up or update an existing MyChart account.
  3. Employee FAQ – Attached document will answer employee questions and provide a simple understanding about the COVID-19 vaccine
  4. Pfizer and Moderna Fact Sheets – More clinical information regarding the vaccines.
