Geriatric Trauma Program

Geriatric patients who experience trauma can require intensive resources. St. Luke’s trauma department has identified an opportunity to improve care for the geriatric trauma patient, including reduced delirium and decreased hospital length of stay (HLOS). All Geriatric trauma patients 65 years of age or older are screened by trauma staff using an evidenced based tool, Identification of Seniors at Risk (ISAR). Geriatric patients who yield greater than or equal to two during this screening receive an in-patient geriatric consult. To improve continuity in care and patient safety, recommendations made in the geriatric consultation - including post-discharge follow-up with a geriatrician - is communicated to the patient’s primary care physician.
