Schedule Your Vaccine Appointment
Vaccines are available for individuals ages 6 months and older, who have a pediatrician or primary care provider through SLUHN. Appointments are required for all vaccines. To schedule, call your St. Luke's provider.

All minors require a completed Minor Consent Form. Please download, complete and bring form to appointment. Forms will be also be available at time of appointment.
Individuals ages 6 months and older who are interested in COVID vaccination should first complete the primary series of the vaccine.
Individuals ages 5 years and older may then consider a booster dose if/when eligible.
Eligible individuals interested in receiving a booster should consider the following:
- Individuals ages 5 years to 11 years should receive the original monovalent booster.
- Individuals ages 12 years to 17 years should receive the updated Pfizer bivalent booster.
- Individuals ages 18 years and older can receive either the updated Pfizer or Moderna bivalent booster.
Pfizer vaccines are available at St. Luke’s for individuals who have a pediatrician or primary care provider through SLUHN. For individuals seeking a Moderna vaccine, please visit vaccines.gov to find a nearby retail pharmacy for vaccine administration.
