6 months to less than 5 years old
5 years and older
Please call your St. Luke’s primary care provider or pediatrician to schedule your vaccine appointment.
All minors require a completed Minor Consent Form. Please download, complete and bring form to appointment. Forms will be also be available at time of appointment.
After completing your primary series of the COVID-19 vaccine, individuals may be eligible to receive a booster or third dose.
Booster shots are available for individuals 5+ who received their Pfizer COVID vaccine at least 5 months ago and individuals 18+ who received their Moderna COVID vaccine at least 5 months ago.
A second booster dose is available for adults who are immunocompromised or are over the age of 50.
Third doses are available for individuals 5+ who are immunocompromised and received their second dose greater than 28 days ago. Pfizer is available for individuals 5+ and Moderna for individuals 18+.
