Community Health Initiatives

Healthy Living and Chronic Disease Prevention Initiatives’ overarching goal is to improve nutritional status and physical activity levels while reducing the chronic disease burden in the St. Luke’s University Health Network service area by creating a culture that supports health living. Initiatives include School-Based Nutrition Education, Employee Wellness, Community Supported Agriculture, Tail on the Trail, Adopt A School, Diabetes and Flu Prevention Initiative. These initiatives address our CHNA goals related to Promoting Health Lifestyles and Preventing Chronic Disease; Improving Access to Care and Reducing Health Disparities; Improving Mental/Behavior Health; and Improving Child and Adolescent Health.
