Insurance

Village Medical Center participates in a variety of managed care and insurance plans. Please contact our office for a list of plans accepted and additional insurance information.

Referrals

When a referral is needed, it's the responsibility of the patient to obtain the referral as well as to understand your insurance plan's requirements regarding facilities and laboratories. As always, our staff is available for assistance.

Referrals are processed Monday-Friday and require 72 hours notice to be completed.

When calling the referral line, please speak slowly and leave the following information:

  • Patient's name
  • Date of birth
  • Insurance company and ID number
  • Name of specialist and specialty
  • Name of facility if applicable
  • Name of physician who referred the patient
  • Diagnosis or reason for referral
  • Whether this is the patient's initial visit
  • Name of the procedure if applicable

Payment

We believe that quality healthcare should always be solely focused on our patients' health. Not their wealth. It is this belief that drives us to contain medical costs, so we can keep the cost down for you, the patient. To make things easier for you, we request payment at the time of each visit.

We accept Visa, MasterCard, personal checks and cash.
